You can join our NHS practice list if you live within our practice boundary and qualify for NHS treatment.
If you do not qualify for NHS treatment we may still be able to treat you as a private patient if you wish. NHS patients
Please bring in your NHS Medical Card and fill in a registration form at our reception.
Since there is a usually a delay in obtaining records from your previous doctor, we also ask that you fill in medical questionnaire. Should you require prescription medication before your main notes arrive, you need to let us know what you are taking, preferably by bringing in the repeat prescription slip from your last doctors. If you do not have this, we will need to see the actual medications you take. You will usually need to book in to see the doctor before receiving repeat medication. Temporary NHS patientsIf you are a resident of the U.K. and if you will be in our area for less than three months, you are probably entitled to join the practice as an NHS temporary resident. Private patients
If you are not a U.K. resident an are not entitled to NHS treatment, you will be able to register as a private patient.
Receipts will be issued to enable you to claim against insurance.
