How to register

To register as a patient with Shay Lane Medical Centre you must live within our practice boundary.

 

Patients can register using the NHS GP Registration Service: https://gp-registration.nhs.uk/P91008

Alternatively, you can print and complete the New Patient Form and Patient Health Questionnaire or request these from reception.

 

If you are taking medication, this information will usually be transferred to us electronically. However we may need to see evidence of your most recent prescription. Please ensure where possible you have enough medication from your last GP, in order to give us time to process your registration and issue your next prescription.

You will have a named, accountable doctor who is responsible for coordinating your care. You can still talk to or make appointments to see any of our doctors or nurses, not just your named GP.

Temporary patients

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient please contact the local practice you wish to use. For example, if you are studying at university and were previously registered with us, you are also seen as a temporary resident. If you are not registered with Shay Lane Medical Centre but are a temporary resident within the practice boundary you will have to complete a Temporary Services Form. Please speak to reception for further information.
